Ive only seen this occur at the 2 small islands at monsters borough. Sea creatures will spawn or get stuck some other way underneath the islands.

I have the same thing in void storm mainly, the hammersharks often spawns here. I always see like 2-3 or 6 inside that island. I think the inside of the island is make of water and that let sharks spawn inside.

Monsters in Monsters Burrow are spawning incorrectly on this island, it is the tall island with the two tall rocks. They are spawning under the sand in groups of 5+

found about 4/5 orcas stuck behind a wall/rock underneath the island closest to eruption, happened earlier as well when i was trying to kill moby on a different server
